New To Magnolia?

What do I need to do before my first visit?

We welcome you to complete the required forms in your New Patient Registration Packet (download below) and bring them with you.  Or you may complete the forms when you arrive at the health center for your visit.  Please arrive at least 15 minutes early to your appointment and please bring your insurance card, a photo ID, and a current medication list.

What payments am I responsible for?

Patients are responsible for co-payments, unmet deductibles, and non-covered services at the time of their visit.  Payment in full is also expected at the time of your visit if Magnolia Family Health does not participate with your insurance plan.

Are telehealth appointments an option?

Through audio and video over the internet, you can meet with your clinician on-the-go from your desktop, laptop, tablet, or mobile device (iOS or Android).


Telehealth allows us to connect anywhere with secure and convenient appointments that save you time and hassle. There's no need to deal with traffic when you can schedule and attend your appointments directly from a laptop or mobile device.
